Bathing Release Form

Submitting this form does not mean your bathing reservation is confirmed. Please use our Appointment Request form and a member of our staff will reach out and confirm your pets bathing appointment.

To ensure that your pet’s bathing experience is as pleasant as possible, please read and fill out the following:

For a safe and pleasant bathing experience, we would like to make you aware of the following bath policies:

Vaccines/Fecal Test – Rabies, distemper/parvo, and canine influenza must be current within the last year, and bordetella current within the last six months. A fecal test for internal parasites must also be current within the last year.

Parasites – Any pet entering the facility with fleas, ticks, or intestinal parasites will be treated by the veterinarian on duty to ensure our environment remains safe for all animals on the premises. The pet owner is responsible for the cost of this treatment.

    Drop Off Illness Policy: *
    In the event of a life or death emergency and your pet becomes ill or a medical condition arises, our Doctors and Technicians will always make every attempt to save your pet's life. We will always make every effort to get in contact with you immediately. In the event that your pet develops a non-life threatening illness, but we would recommend a Doctor exam and treatment, please indicate what you would like for us to do while your pet is here in the Hospital.

    Our bathers work very hard to have your pet bathed and dried in a timely manner. You will be notified when your pet is ready to go home. Some pets may take longer than others, so if you need to pick up at a certain time, please let us know at drop off.

    I assume all financial responsibility for charges incurred relating to my pet's treatment and care. I understand that payment in full is due at the time my pet is discharged.
